LEARNING OUTCOMES

The aim of the course is to provide the theoretical framework on which the nursing care of the chronically ill individual can be based; to enable students to identify specific aspects of their role in the rehabilitation health care team, to be able to relate theory to practice and acquire the skills needed to care for the chronically ill individual and rehabilitation.

After the end of the course students will be able to:

  • Understand the concept and the basic principles of chronic disease / disability management and rehabilitation.
  • Describe the causes of social exclusion of people with chronic illness / disability and analyze ways to address social discrimination
  • Understand the goals and obstacles in promoting health and teaching patients with chronic diseases / disabilities and acquire the appropriate knowledge and skills to promote the health and well-being of these individuals.
  • Be able to teach self-care skills to people with chronic illnesses and contribute to their rehabilitation in the community.
  • Describe the role of the nurse, in the management and rehabilitation of the chronically ill / disabled individual , as a liaison person  within the interdisciplinary health care team.

Course Outline

 

SYLLABUS

  • Basic concepts of chronic illness, disability, physical, occupational and social rehabilitation. The concept of nursing rehabilitation and care. Characteristics and qualifications of the rehabilitation nurse.
  • Nursing theories and models of nursing care and rehabilitation of chronically ill individuals. Enhancing  self-care for  the chronically ill individuals.
  • Stress, illness and recovery.
  • Ethical and Legal Framework for Disability -Description of Rehabilitation Centers- Natural and artificial means used in rehabilitation.
  • Management and rehabilitation of people with chronic diseases (Management of common symptoms of people with cardiovascular and respiratory diseases (CKD and COPD), neurological diseases (SLE, Dementia), spinal injuries, autoimmune diseases, depression, kidney diseases)
  • Metabolic diseases and patient education for self-management of the disease in the community.
  • Case study and development of care plans for patients with chronic diseases and mobile disabilities.
  • Nursing assessment and use of disability assessment tools.
  • Physical activity-Promoting mobility and physical activity, Dietary Interventions-Promoting swallowing in chronically ill patients.
  • Bladder and bowel function test-Skin integrity.
  • Evaluation and management of pain and fatigue in chronically ill patients.
  • Psychological and social problems of patients with disabilities and nursing treatment of fear, anger and frustration. Counseling of the chronically ill individual and his family.
  • Secondary and tertiary prevention of chronic diseases in the community-Health promotion and teaching skills of self-management and adaptation to disability.
  • Nursing role in the health, rehabilitation team-Interprofessional cooperation.
